Assessment Questionnaire

Assessment Questionnaire
About
  • Ages and Stages Questionnaire
  • Butterfly Kisses Childcare uses the Ages and Stages Questionnaire to assess children in our care. Screening with ASQ is a powerful first step to understanding how a child is progressing through his or her developmental milestones.
  • Parents, educators, and professionals work together to gain critical knowledge that helps them determine what to do next. ASQ’s assess children on their Fine Motor, Gross Motor, Communication, Personal-Social, Problem Solving, and Social- Emotional Skills.
      About
      • We assess children three times a year, in November, April, and August. We find these times of year most beneficial and accurate to assess your child’s progression through his or her developmental milestones. Children begin each school year with a new teacher. The time between when they begin in their new room and November is an adequate amount of time for the caregivers to become familiar with your child.
      • We assess again in April as we approach an end to a school year and we finally assess in August. August is the last month they will spend in their classrooms before moving up.
          About
          • For children on track, ASQ results offer reassurance and the opportunity to celebrate new milestones. For children who are not meeting milestones, screening alone isn’t complete without the final step of referral—whether you make a formal referral for a child who qualifies for state and federal programs or a referral to local resources and services.
          • Guardians may choose to work one on one with the teacher and work on an individualized plan for additional support if they prefer not to seek outside sources. The purpose of this screening is to see that young children who have developmental delays get the intervention services they need.
              About
              • Quarterly Progress Reports
              • Quarterly Progress Reports are a brief summary of your child’s assessment in the past three months. Knowing how your child is acclimating to their classroom is important to all parents. We want to make sure you have access to your child’s individual progress.
              • Along with information about your child’s progress, we will continue to inform you on what the teachers will be further working on and strategies to help promote your child’s growth.
                  About
                  • Preschool Evaluations
                  • Our Three Year Old and Four Year Old Preschool programs biannually send home a brief report on each child to inform parents on how their child is developing academically. This is additional to our Progress Reports and ASQ’s.
